Registered Nurse - Home Health

Job Description

Job Summary:
Provide comprehensive home health nursing care to patients in their community setting, ensuring high-quality clinical support and adherence to care plans. Responsible for patient assessments, education, and coordination with healthcare teams to promote positive health outcomes.

Responsibilities:

• Administer personalized patient care according to physician orders and established protocols.
• Conduct thorough patient assessments, including OASIS data collection when applicable.
• Educate patients and families on disease management, medication adherence, and health maintenance.
• Document clinical findings and care activities accurately into electronic health records.
• Collaborate with interdisciplinary teams to develop and modify patient care plans.
• Ensure compliance with all regulatory and safety standards, including timely completion of required assessments and documentation.

Qualifications:

• Valid RN license with pending license accepted, with ability to obtain prior to start date.
• Minimum of 1-2 years of recent home health nursing experience preferred.
• Strong knowledge of home health protocols, OASIS, and documentation requirements.
• Ability to pass required clinical tests including Cardiac Dysrhythmia and Moderate Sedation exams.
• Excellent communication and patient education skills.
• Must have current BLS certification.
