Family Medicine PhysicianThe family medicine physician provides primary care services to patients of all ages within our outpatient clinic that specializes in I/DD. This includes developing and implementing a plan of care, prescribing medications, and appropriately documenting each patient encounter in the electronic health record (EHR).Key/Essential Functions & Responsibilities:Provide evidence based primary care services to both pediatric and adult patientsOrder and interpret diagnostic testsRefer patients for specialty care when indicatedPartner with other members of the care team (e.g., nurse, medical assistant, social worker)Report and coordinate care with Medical DirectorComplete all EHR documentation and administrative tasks in a timely mannerMinimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g., lifting, assisting lifting, standing, etc.):MD or DO degreeCompletion of an ACGME-approved family medicine residency programBoard certified or board eligible in family medicineValid DEA registration and New York medical licenseBLS certificationPreferred Qualification Requirements (desired requirements beyond MQRs above):Fluency in Spanish2+ years of experience as a primary care physicianExperience with the I/DD patient populationActive Medicare and NYS Medicaid registrationFamiliarity with the NextGen EHRCompensation:Compensation is from 160000 USD to 180000 USD per yearAll qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, age, disability, or status as a Vietnam or disabled veteran.
