We support patients and their families with dignity, comfort, and love. What You’ll Do Serve as the primary point of contact for patients and families through the entire hospice journey. Deliver and document skilled, hands‑on nursing care based on each patient’s individualized Plan of Care in the home, assisted living, or other settings. Perform initial and ongoing assessments to monitor patient condition and adjust care as needed. Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to develop and refine personalized care plans. Provide emotional, physical, and spiritual support to patients and their loved ones, offering education, comfort, and bereavement guidance. Teach caregivers how to safely care for their loved one with both practical training and emotional reassurance. Communicate important updates in patient condition to the care team to ensure timely and appropriate interventions. Maintain accurate, up‑to‑date clinical documentation and adhere to infection control and compliance standards. Support the organization’s quality improvement and scheduling initiatives to keep care consistent, compassionate, and efficient. Help patients and families navigate medication administration, hospice philosophy, symptom and pain management, and end‑of‑life processes. Compensation Salary Range: $98,000–$105,000 . Compensation may vary within the provided range based on location, experience, education, certifications, licensure, and department equity. Hospice or palliative care experience preferred. Licenses & Certifications Current state nursing license CPR Certification Valid driver’s license and automobile insurance (required for travel) Working Conditions Patient homes and/or institutional settings and automobile travel. Potential exposure to infections, communicable diseases, odors, blood, excreta, and hazardous materials. Personal Traits Sensitivity to life‑and‑death issues and ability to support others in crisis. Flexibility to perform varied tasks without loss of efficiency or composure. Effective communication with people of all socioeconomic backgrounds. Responsible, mature, cooperative, and tactful in interactions with colleagues and the hospice team. Equipment & Tools Effective operation of nursing assessment tools, computers, facsimile equipment, copier, and cell phone/beeper. Maintain personal transportation for work assignments.
