LTSS Care Manager: Waiver & Care Coordination
Molina Healthcare of Illinois
Molina Healthcare seeks a Care Coordinator to support LTSS-specific care management and coordinate member care across the continuum. You will work with a multidisciplinary team to ensure progress toward outcomes and align with strategies for quality and cost-effectiveness.
