What You'll Do

As a Registered Nurse , you'll provide clinical nursing support within an outpatient psychiatric setting, with a primary focus on safe medication administration, patient monitoring, education, documentation, and coordination of care.

???? Medication Administration & Patient Care

Your responsibilities include:

• Administer long-acting injectable (LAI) antipsychotic medications according to prescriber orders, manufacturer guidelines, and clinic protocols.
• Use appropriate dilution/reconstitution procedures, needle gauge, and Z-track or deltoid/gluteal injection techniques as indicated.
• Verify patient identity, medication, dose, injection site, and due date against the Medication Administration Record (MAR) before each injection.
• Perform pre-injection assessments, including psychiatric status, current medications, allergies, injection-site condition, symptoms, and side effects.
• Maintain and monitor injection schedules for an assigned patient panel.
• Proactively contact patients who are due or overdue for injections to support treatment adherence and relapse prevention.
• Observe patients for immediate post-injection reactions and initiate emergency protocols when necessary.
• Monitor and document medication side effects, including EPS, akathisia, tardive dyskinesia, sedation, metabolic changes, and injection-site reactions.
• Promptly escalate clinical concerns to the prescribing provider.

???? Clinical Monitoring & Documentation

• Coordinate metabolic and other required laboratory monitoring, including weight, glucose, lipids, and prolactin, according to provider orders and clinic protocols.
• Accurately document all injections administered, including medication, lot number, dose, site, and patient response.
• Maintain accurate Electronic Health Records (EHR).
• Provide patient and caregiver education regarding the purpose, benefits, potential side effects, and importance of maintaining the LAI treatment schedule.
• Maintain accurate medication and supply inventory.
• Ensure appropriate storage, refrigeration, expiration-date tracking, and disposal of medications and supplies.
• Handle and dispose of biohazardous materials and sharps according to OSHA and clinic safety standards.

???? Care Coordination & Team Collaboration

• Coordinate with pharmacies and prescribers regarding prior authorizations, medication procurement, and timely refills.
• Consult the Psychiatrist/Medical Director regarding cases outside the nurse's scope of practice, missed or refused injections, or signs of clinical decompensation.
• Communicate with case managers, care coordinators, and outpatient treatment teams to support continuity of care.
• Maintain patient confidentiality and comply with HIPAA and applicable behavioral health confidentiality regulations, including 42 CFR Part 2.
• Use trauma-informed, de-escalation, and therapeutic communication techniques when working with patients experiencing agitation, psychosis, or distress.
• Demonstrate professional, cooperative behavior with patients, providers, and colleagues.
• Perform other duties as assigned by the Director or Medical Director.

???? QualificationsEducation, Licensure & Experience

• Current, unrestricted Registered Nurse (RN) license in Pennsylvania
• Current CPR/BLS certification required
• Training and competency in administration of long-acting injectable antipsychotic medications required prior to independently administering treatments
• One year of clinical nursing experience required
• Psychiatric, behavioral health, or community mental health experience strongly preferred
• Working knowledge of antipsychotic medications, common side effects, and psychiatric monitoring requirements
• Training or experience in de-escalation and trauma-informed care preferred

• Ability to build rapport with patients experiencing serious mental illness
• Ability to work independently and as part of a multidisciplinary psychiatric care team
• Strong attention to detail
• Ability to multitask in a fast-paced outpatient clinic environment
