Registered Nurse (RN) – Hospice Case Manager

Location: Washington, Chisago, & Anoka Counties, MN
Schedule: Monday–Friday | 8:00 AM – 5:00 PM

Job Overview

We are seeking a compassionate and experienced Registered Nurse (RN) with a strong hospice background to join our growing team. In this role, you will provide comprehensive patient assessments, coordinate interdisciplinary care, and advocate for patients and their families to ensure they receive the highest quality end-of-life care.

• 8:00 AM – 5:00 PM

License Requirements

• Active, unrestricted Minnesota Registered Nurse (RN) license

Mileage Reimbursement

• $0.50 per mile

Territory Covered

• Washington County
• Chisago County
• Anoka County

Required Experience

• Hospice experience is required

Key Responsibilities

• Complete comprehensive assessments of hospice patients.
• Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary care team.
• Coordinate care transitions and ensure patients receive the appropriate level of care.
• Work closely with physicians, social workers, discharge planners, and other healthcare professionals to optimize patient outcomes.
• Arrange non-medical support services, including transportation and community resources, to improve treatment compliance.
• Continuously evaluate clinical, functional, and financial outcomes throughout the care management process.
• Identify opportunities for health promotion, symptom management, and illness prevention.
• Proactively prevent adverse patient events and intervene quickly when necessary.
• Maintain accurate and timely clinical documentation.
• Serve as a patient advocate while protecting privacy and confidentiality.

Qualifications

• Associate's or Bachelor's degree in Nursing or a related field.
• Current, unrestricted Minnesota RN license.
• Required: Previous hospice nursing experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, organization, and care coordination abilities.
• Ability to analyze complex medical information and make sound clinical decisions.
• Passion for providing compassionate, patient-centered end-of-life care.
