• Conducting thorough evaluations of patients both in inpatient and outpatient settings, and crafting tailored treatment plans that suit individual needs.
• Implementing physical therapy interventions aimed at improving mobility, enhancing strength, and fostering balance and functional recovery.
• Collaborating closely with a multidisciplinary team composed of nurses, physicians, case managers, and fellow rehabilitation specialists to ensure optimal patient outcomes.
• Educating both patients and their families on therapeutic objectives, plans for discharge, and home exercise routines.
• Meticulously documenting evaluations, treatment procedures, and patient's progress in accordance with facility protocols and regulatory standards.
• Engaging in care team discussions to promote a seamless and coordinated approach to care.

To qualify for this fulfilling position, we require:

• A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution.
• Valid state licensure as a PT / Physical Therapist, with CPR certification being a preferred asset.
• Any specialized certifications (neurological, geriatric, etc.) will be viewed as a plus, but are not mandatory.

In terms of skill set, we are looking for:

• Solid prior experience in the field is strongly preferred.
• A demonstrated ability to care for patients facing complex medical and mobility challenges.
