Licensed Practical Nurse

Kingston Healthcare is seeking a dedicated Licensed Practical Nurse to join our Healthcare & Medical Services team at Kingston Health Center of Vermilion in Vermilion, OH. In this role, you will provide high-quality, resident-centered nursing care in an assisted living, rehabilitation, and memory care environment. Working under the direction of registered nurses and physicians, you will administer medications and treatments, perform clinical assessments and observations, document care accurately, and help develop and follow individualized care plans that support residents’ physical, emotional, and social well-being.

As an LPN at Kingston Health Center, you will monitor vital signs, recognize and report changes in resident conditions, and coordinate with interdisciplinary team members to ensure timely interventions. You will assist with wound care, injections, catheter care, and other skilled nursing procedures within your scope of practice. You will also provide education and emotional support to residents and their families, helping them understand treatment plans and care goals. A key part of your role is to promote safety, infection control, and compliance with all state and federal regulations, as well as facility policies and procedures.

You will work closely with nursing assistants and other care team members, providing direction, supervision, and feedback to support high standards of care. Strong communication and collaboration skills are essential, as you will regularly interact with physicians, therapists, social workers, and other healthcare professionals to coordinate services. You will contribute to care conferences, update documentation in electronic health records, and participate in quality improvement initiatives aimed at enhancing resident outcomes and satisfaction.
