RN Case Manager - Home Health

Location: Plainsboro and West Windsor, NJ

Assignment Duration: This is a long-term contract position.

Schedule: Shift: 8:00 AM - 4:30 PM

Job Description: We are seeking a dedicated RN Case Manager for our Home Health division. The selected candidate will join our South team and be responsible for managing a caseload of patients, conducting assessments, and ensuring comprehensive care delivery in a home health setting. This role is suited for an experienced nurse with a background in case management and OASIS documentation.

Responsibilities:

• Manage a caseload of 5-6 patients per day.
• Conduct assessments and develop care plans in collaboration with patients and their families.
• Document patient information accurately and thoroughly, ensuring compliance with OASIS requirements.
• Provide wound care as needed, utilizing advanced nursing skills.
• Coordinate with multidisciplinary teams to enhance patient care.

Required Experience / Certifications / Licensure:

• 2+ years of experience in home health care.
• Strong case management skills and experience with OASIS documentation.
• Preferred experience in wound care.
• Active RN license in the state of New Jersey.
