How you'll contribute Manages drug therapy protocols as a part of an interdisciplinary team. Reviews, interprets and evaluates medication orders to identify, prevent and resolve potential or actual drug related problems. Provides drug information and education to medical staff, healthcare professionals, and patients, and pharmacy students. Documents interventions and their outcomes for use in drug therapy quality improvement efforts. Verifies medication orders for accuracy/completeness prior to entry; checks filled medications prepared by technicians; maintains proficiency with pharmacy automated systems and software. Manages patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling. Participates in performance improvement and required continuing education. Performs other duties as assigned. What we're looking for Applicants should have a current state Michigan Pharmacy License and possess a doctorate's degree from an accredited school of pharmacy. Additional requirements include: Education: Doctor of Pharmacy License: Current license with the state Board of Pharmacy. Certifications: BCLS required; ACLS preferred.
