Minimum Requirements Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active Connecticut Physical Therapist license is required to start the assignment. BLS certification from American Heart Association or American Red Cross may be required. Demonstrated experience working in a skilled nursing facility or long-term care setting preferred. Experience with post-acute care evaluations, restorative programs, and discharge planning preferred. Proficiency with common therapy equipment including gait belts, therapeutic modalities, and durable medical equipment preferred. Ability to document in electronic medical records and meet productivity and regulatory requirements preferred. Assignment Details Facility type: Skilled Nursing Facility. Shift: Days.
