Job Responsibilities

• Conduct thorough evaluations in inpatient or outpatient environments, creating personalized treatment plans tailored to individual needs
• Administer diverse physical therapy interventions enhancing mobility, strength, agility, and overall functional performance
• Collaboratively engage with nursing staff, physicians, case managers, and rehabilitation professionals to optimize patient outcomes
• Educate patients and their families on their therapeutic journey, discharge protocols, and home exercise regimens
• Accurate documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Participate in multidisciplinary rounds and conferences to facilitate well-rounded care

Requirements

• Doctorate in Physical Therapy (DPT) or a similar recognized degree from an accredited institution

Certifications

• Active State licensing as a Physical Therapist
• CPR certification preferred
• Other specialized certifications (e.g., in neurology, geriatrics) are welcomed but not necessary
