LVN - Long-Term Care Nurse & Care Coordinator

Sweetwater Care

Sweetwater Care is seeking a licensed LVN/LPN to provide nursing care within the scope of practice, ensuring patient needs are met in accordance with standards, physician orders, center policies and applicable guidelines. The role requires long-term care experience, familiarity with PointClickCare charting, and current licensure in the practicing state, plus BLS/CPR certification or willingness to obtain. You will document changes in patient conditions and participate in physician rounds as #J-18808-Ljbffr Sweetwater Care
