About the Role

We are currently seeking a compassionate and skilled Home Health Caregiver to join our team. As a Home Health Caregiver at Modani Care, you will have the unique opportunity to make a meaningful difference in the lives of others while working in a rewarding and dynamic environment. Your primary responsibilities will include:

• Providing high-quality, compassionate care to patients in their homes, with a focus on assisting with daily living activities, such as bathing, dressing, and grooming
• Developing and implementing personalized care plans in collaboration with patients, families, and healthcare providers
• Maintaining accurate and detailed records of patient care and treatment
• Communicating effectively with patients, families, and healthcare providers to ensure seamless care transitions
• Promoting patient independence and autonomy while prioritizing their safety and well-being

Requirements
• Valid driver's license and reliable transportation (for travel to and from patient visits)
• Ability to lift, transfer, and move patients as needed
• Strong communication and interpersonal skills
• Compassionate and empathetic demeanor, with a commitment to providing high-quality patient care
