POSITION DETAILS Job Title : Community Health Worker
Salary : $21.63/hr (Paraprofessional 1)
- Salary is based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related reasons
- LPHI offers traditional (M-F, 8 am-5 pm), flexible (for example, M-F 10 am-7pm), and compressed work schedules (for example, M-Th 8 am-7 pm). Schedule details finalized between employee and manager.
- LPHI maintains an office in New Orleans if the employee prefers to be fulltime in-office
- Employee must reside in Louisiana during the time of employment
- Exercise good judgment with respect to priorities and effective use of the team's time.
- Develops a deep understanding of the programmatic areas and partners to support administrative improvements.
- Maintain files of relevant organizational and programmatic documents. Updates documents, as necessary.
- Engage in professional development activities to improve job-related skills and knowledge.
- Outreach and Education: Conduct outreach activities to educate community members about preventive health measures and available services using, virtual, in-person, or hybrid platforms.
- Healthcare Navigation: Assist individuals in navigating the healthcare system, including helping them access and making referrals for medical care, social services, and emergency assistance.
- Chronic Condition Support & Management: Provide support to individuals managing chronic conditions and advocating for their needs.
- Data Collection: Support documentation management efforts to Gather and relay community health data back to healthcare organizations to ensure that programs and policies are responsive to community needs.
- Internal Communication: Communicate and coordinate across internal teams and with external partners to gain an understanding of internal and community assets to broaden perspective, and to continue to grow and learn.
- Coordination: Coordinate and facilitate meetings, trainings, and events using virtual, in-person, or hybrid platforms as required.
- Conduct coordination activities and support for projects, including: Developing communications materials, including e-mails to partners, website updates, promotional products, and more.
- Collaboration: Proactively collaborate with team members to ensure tasks are completed. Participate in organization, department, and project level trainings and meetings.
- 3-5 years of experience as a Community Health Worker, supporting community care coordination required.
- Knowledge of public health, chronic conditions, and regional healthcare navigation.
- Strong interpersonal and relationship-building skills with experience working with individuals and groups of diverse backgrounds.
- Ability to think proactively, be a self-starter, and function both independently and as a team.
- Ability to collaborate effectively and build strong, positive relationships with colleagues and partners, both internal and external to LPHI.
- Exceptional emotional intelligence, including interpersonal skills and experience working with individuals and groups from diverse backgrounds.
- Demonstrated learning orientation; ability to learn new subject areas quickly.
- Ability to champion and manage change.

- Ability to utilize virtual meeting platforms, including but not limited to Microsoft Teams and Zoom to participate and coordinate meetings with clients and activities in a virtual environment.
