In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Minneapolis, MN

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. Job highlights The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and additional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the person's health plan. Evaluations are performed using an iPad and an intuitive clinical workflow. Clinicians are not required to prescribe medicine, order lab tests, or alter treatment regimens. In this role, you will: Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes. Contribute to the goal of visiting millions of people nationwide to provide connections to the right care for each individual's unique needs. Work with Signify Health as a clinician colleague, fulfilling the responsibilities of the specified role. Be flexible to travel locally and within licensed states, as needed. Conduct virtual visits as needed. Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening). A Note on Our Work Environment As an in‑home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must comfortably and safely perform their duties in such settings.
