Schedule

8am to 4pm, Tuesday to Saturday or Sunday to Thursday. Some on-call and holiday shifts required.

Job Description

The Registered Nurse will utilize the nursing process to assess patient and caregiver needs, ensuring quality care in compliance with evidence-based practices and relevant state and federal regulations. Responsibilities include developing effective care plans, serving as a case manager, completing OASIS documentation, and implementing nursing interventions.

Responsibilities

• Assess patient and caregiver needs, ensuring compliance with practice standards.
• Develop and communicate realistic plans of care, including patient involvement.
• Manage case loads effectively in alignment with agency productivity standards.
• Complete OASIS documentation accurately and in a timely manner.
• Implement safe and effective nursing interventions as per medical plans.
• Stay updated on high-tech skills and infection control standards.
• Collaborate with LPNs and ensure proper delegation of tasks.
• Evaluate and document the effectiveness of implemented care plans.
• Maintain accurate documentation for ongoing quality patient care.
• Follow agency protocols and McKesson processes for efficient operations.
• Educate patients and caregivers regarding health matters and treatment options.
• Identify and document interdisciplinary needs for comprehensive patient care.
• Fill out relevant agency, state, and federal forms accurately and promptly.
• Manage visits effectively, ensuring supervisory visits are properly scheduled.
• Coordinate patient care and ensure safe discharges and transfers.
• Participate in monthly peer review audits and provide feedback.
• Facilitate case conferences to enhance team communication and care delivery.
• Advocate for patient care issues to ensure tailored and sensitive care.

Required Experience / Certifications / Licensure

• Current RN license in the state of practice.
• Experience with OASIS documentation is required.
• Knowledge of evidence-based practices in nursing care.
