What You'll Do As the MDS RN Coordinator, you'll help ensure each resident receives the individualized care they deserve by working closely with our interdisciplinary team. Responsibilities include: Coordinating and completing MDS assessments. Leading the interdisciplinary care planning process. Partnering with nursing, therapy, dietary, social services, and physicians. Monitoring assessment schedules and regulatory compliance. Assisting with admissions, evaluations, and care transitions. Supporting quality improvement initiatives. Serving as a resource for reimbursement and assessment processes. We're Looking For Someone Who Holds a current RN license. Has strong assessment and critical thinking skills. Communicates well with residents, families, and coworkers. Enjoys collaborating with others. Is organized and detail-oriented. Leads with compassion, integrity, and professionalism.
