Job Description
Case Manager RN

We are seeking a dedicated and qualified Case Manager RN to join our team. The ideal candidate will possess an active AK RN license and have a minimum of one year of relevant experience. This position offers a 4x10 day shift schedule.

Key Responsibilities:

Provide high-quality care coordination for patients.
Utilize Epic for patient documentation and management.
Collaborate with healthcare team members to ensure appropriate care plans are implemented.
Communicate effectively with patients and their families regarding care decisions.
Attend and facilitate care meetings as needed.
Conduct assessments and evaluations to determine patient needs.

Qualifications:

Active Alaska Registered Nurse (RN) license required for auto offer.
Basic Life Support (BLS) certification required.
Acute Care experience is essential.
ACM certification is preferred but not required.
Experience with Epic electronic health record system is mandatory.
Must be able to float within scope to meet facility needs.

Schedule & Requirements:

Shift: 4x10 Days, with start time around 07:00 AM.
Weekend commitments are required every other weekend or a total of four weekend days within a four-week schedule.
RTO restrictions apply: only one time-off request per 13-week assignment of 5 days or less, with no holiday time-off allowed for auto offers.
Guaranteed hours: 70 hours per 2 weeks; travelers may be called off for one 10-hour shift within the 2-week period.
Local candidates are not accepted; applicants must reside at least 50 miles from the facility.
