In-Home Nurse Practitioner: Flexible Contractor
Signify Health
Signify Health, part of CVS Health, is seeking board-certified nurse practitioner contractors to conduct in-home health evaluations for Medicare Advantage, Medicaid, and select plan members. You will visit members at home, review medications and histories, and complete a patient-specific evaluation form.
