PRN Registered Nurse (RN) or Licensed Practical Nurse (LPN)

South Bend & Elkhart, IN

Current Opportunity

We are seeking a PRN Registered Nurse, preferably, or Licensed Practical Nurse to provide intermittent skilled nursing visits in the South Bend area , with the ability to assist with visits in Elkhart when needed.

An RN is preferred to assist with patient admissions; however, qualified LPN candidates are also encouraged to apply.

Current Scheduling Needs

• Two patient visits every other Friday

• One patient visit every other Sunday

• Additional PRN visits may become available as patient needs grow

Nursing Responsibilities

• Provide intermittent skilled nursing visits in patients’ homes

• Complete nursing assessments and monitor changes in patient condition

• Follow individualized plans of care and physician orders

• Provide education and support to patients and their families

• Maintain accurate and timely clinical documentation

• Communicate with physicians, care coordinators, and other members of the care team

• Assist with admissions and start-of-care visits when qualified and needed

• Travel to South Bend and occasionally Elkhart for scheduled visits

Qualifications

• Active Indiana RN or LPN license in good standing

• RN preferred, particularly for admission visits

• Previous home health, skilled nursing, or geriatric care experience preferred

• Strong assessment, communication, and documentation skills

• Dependable, compassionate, and professional

• Reliable transportation and a valid driver’s license

• Ability to pass all required background screenings
