Schedule / Expected Work Hours:

• This is a part-time clinical supervisor support position with flexible scheduling. The Agency's normal business hours are Monday through Friday, 9:00a - 5:00p; however, this position is not expected to work a standard Monday-Friday, 9:00a-5:00p schedule .
• We are primarily seeking availability during evenings and weekends , with occasional availability during normal business hours as needed.
• This position will provide additional coverage and support to the Clinical Supervisor , including assisting with client visits and clinical needs when the Clinical Supervisor is unavailable or has a high workload.
• Participation in a clinical on-call rotation (phone support only) may also be required.

Job Summary

Manages, supervises, coordinates, evaluates, and develops client care teams to ensure quality care delivery within the scope of services of the Agency and all applicable

policies, standards, laws, and state Nurse Practice Act.
Reports To: Director of Clinical Services/Administrator
Provide Supervision to: All direct care staff

Responsibilities

1. Conduct in-home client visits such as start-of-care assessments, supervisory visits, and reassessment visits.
2. Be available during assigned hours and on-call as needed to support quality care delivery.
3. Receive referrals, assess service needs, and assign appropriate clinicians to cases.
4. Meet regularly with clinical staff to provide guidance, set goals, and evaluate progress.
5. Attend clinical meetings to facilitate care coordination and participate in on-call rotation.
6. Support staff, clients, and families by providing direction on healthcare resources and educating them on clinical policies.
7. Oversee accuracy and timeliness of clinical documentation and perform quarterly chart reviews.
8. Assist with staff hiring, orientation, training, and conducting annual performance evaluations.

Qualifications

• Graduate of an accredited school of professional nursing. BSN preferred.
• Current license as a Registered Nurse in the state(s) of practice having one (1) year nursing experience within the last three (3) years, at least two (2) years of homecare experience and one (1) year of supervisory experience.
• Excellent verbal and written communication skills and strong interpersonal skills.
• Must be a licensed driver with an automobile insured in accordance with Agency and state requirements.
• Current CPR certification.

Compensation: $37.00 per hour
