As a Home Health Registered Nurse, you'll provide personalized, one-on-one care that helps patients recover from illness, manage chronic conditions, and maintain their independence while receiving care in the comfort of their own homes. What You'll Do Perform comprehensive nursing assessments and skilled visits Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments Develop and implement individualized plans of care Provide skilled nursing services, including medication management, wound care, disease management, and patient education Monitor patient progress and communicate changes with physicians and the interdisciplinary care team Coordinate care with therapists, social workers, and home health aides Complete timely and accurate clinical documentation using WellSky Educate patients and caregivers to promote safety, independence, and improved health outcomes Qualifications Active Virginia Registered Nurse (RN) License Required Valid Driver's License and Reliable Transportation Strong Clinical Assessment, Critical Thinking, and Documentation Skills Home Health Experience Preferred but Not Required OASIS Experience Preferred Experience with WellSky Home Health is a plus Passion for Delivering Exceptional Patient‑Centered Care Why Human Touch Home Health?
