Key Responsibilities
- Provide skilled physical therapy services to geriatric patients in a home health setting
- Perform and document Start of Care assessments using OASIS
- Manage a daily caseload of up to 6 patients while meeting productivity standards
- Travel independently to patient homes within the assigned service area
- Collaborate with the care team to develop and implement individualized treatment plans
Required Qualifications
- Active California Physical Therapist license
- Minimum of 1 year of recent skilled home health experience (within the last 3 years)
- Proficiency with OASIS documentation, including Start of Care assessments
- Experience with HCHB (HomeCareBiz) electronic medical records system
- Current Basic Cardiac Life Support (BCLS) certification
- Must possess reliable personal transportation
Preferred Experience / Skills
- Experience working with a geriatric patient population in a home health environment
- Familiarity with efficient caseload and route management to minimize drive time
