Position Summary The Community Mental Health Therapist provides direct therapeutic services to children, youth, and families referred through the Medi-Cal system in San Francisco County, including Educationally Related Mental Health Services (ERMHS) and programs supporting justice-involved youth. Services are delivered in community-based settings such as schools, homes, and public service agencies to ensure accessibility and responsiveness to client needs. This position is part of an interdisciplinary care team that collaborates with the San Francisco Department of Public Health (SFDPH), San Francisco Unified School District (SFUSD), Juvenile Probation Department (JPD), and other community partners. The therapist provides trauma-informed, culturally responsive, and strengths-based services that support healing, resilience, and stability for youth and families involved in multiple systems of care. Job Responsibilities Clinical Services Conduct comprehensive biopsychosocial assessments and develop individualized treatment plans that meet Medi-Cal and SFDPH standards. Provide individual, family, and group therapy to youth and families in schools, community centers, and home settings. Deliver ERMHS services and collaborate with educators and school-based staff to support Individualized Education Program (IEP) and academic goals. Offer therapy and care coordination for youth involved with the probation system, emphasizing rehabilitation, growth, and positive development. Maintain accurate and timely documentation in accordance with YMCA, SFDPH, Medi-Cal, and Juvenile Probation standards. Participate in case consultations, interdisciplinary meetings, and clinical supervision to ensure coordinated and effective care. Interdisciplinary Collaboration Collaborate with SFDPH clinicians, probation officers, and care coordinators to provide comprehensive and coordinated client support. Partner with Intake Coordinators to facilitate timely intake, service assignment, and client engagement. Work with teachers, school psychologists, and other service providers to integrate mental health goals with academic and behavioral supports. Support cross-agency communication to promote continuity of care and high-quality service delivery. Community Engagement and Advocacy Conduct outreach to identify youth and families who may benefit from mental health services. Participate in community wellness events, educational workshops, and family support activities that promote emotional well-being. Serve as a liaison between the YMCA and community partners to align services with local and family needs. Administrative and Compliance Complete all required documentation, including progress notes, treatment plans, and discharge summaries, within established timelines. Ensure adherence to HIPAA, FERPA, and other applicable local, state, and federal confidentiality regulations. Attend required meetings, clinical supervision, and professional development trainings. Participate in quality assurance reviews, audits, and data collection to support continuous program improvement. Qualifications Master’s degree (MA/MS) in Counseling, Clinical Psychology, Marriage and Family Therapy, Social Work, or equivalent, or within one academic year of completion. Registration with the California Board of Behavioral Sciences (BBS) (AMFT, ASW, APCC) or eligibility within 90 days of hire. At least two years of experience providing behavioral health services or case management in community, school, or similar settings. Knowledge of trauma-informed and culturally responsive clinical practices. Strong interpersonal, written, and verbal communication skills.li> Ability to collaborate effectively within an interdisciplinary team. Proficiency with Microsoft Office Suite and electronic health record systems (e.g., EPIC, Avatar). Current CPR/First Aid Certification (YMCA-approved) Preferred Qualifications (Optional) Experience working in Educationally Related Mental Health Services (ERMHS) or other school-based behavioral health programs. Experience providing therapeutic services to youth involved in the juvenile justice system. Familiarity with San Francisco County behavioral health systems and community resources. Proficiency in more than one language (such as Spanish, Cantonese, Tagalog, or Samoan) is highly valued to better serve diverse communities. Work Environment & Physical Demands This position involves providing services in schools, homes, and community-based settings throughout San Francisco County. Travel between sites may be required; employees may use public transportation or a personal vehicle. Mileage reimbursement is provided when applicable. The role requires the ability to sit, stand, walk, and engage with youth and families in various community environments.
