What You'll Do:
To act as a liaison between the home, school and community agencies for students who are determined to be at risk for academic failure. Develop and facilitate an action plan between the school and the family to help ensure student's success to include weekly in-school contacts with children and bi-weekly home visits with parents. Act as the attendance and truancy liaison for individuals on the referral caseload list and others as requested.
Responsibilities
Update direct supervisor on an ongoing basis regarding concerns, issues and caseload;
Develop and enhance communication, networking, and collaboration between parents, schools, and community agencies;
Provide supportive services and follow-up activities both in the home and school environments;
Upon receipt of a referral, convene a meeting of all involved parties to coope
