Physical Therapist (PT) - Home Health

Home Health Physical Therapist

Schedule: Days, 5x8 (8:00 AM–4:30 PM)

Assignment Details: Visits per day: ~6

Requirements:

• New Hampshire state PT license (valid or pending with ability to obtain by start)
• BLS certification (acceptable AHA/Red Cross copies only)
• Minimum of 1 year of PT experience required

Roles & Responsibilities:

• Conduct initial home assessments and develop treatment plans.
• Evaluate patient needs and implement interventions for strength training, balance, and fall-risk reduction.
• Provide education to patients and caregivers regarding home exercise programs and equipment needs.
• Monitor patient progress and adjust treatments as necessary.
• Coordinate discharge to the community when appropriate.

Compliance / Notes:

• Orientation: 10 hours
• Flu vaccine required (medical/religious exemptions only)
• Drug screen required within 14 days of start
• Background/license issues must be approved by the facility prior to offer
