Opportunity Highlights

• Schedule: Monday-Friday, 8:00 AM - 5:00 PM; 2 weeks per month ongoing. Specific dates include:
  • Nov 30 - Dec 4
  • Dec 21 - 24
  • Dec 28 - 31
  • Jan 25 - 29
  • Feb 1 - 5
  • Feb 8 - 12
  • Feb 15 - 19
  • Feb 22 - 26
  • Mar 1 - 5
  • Mar 8 - 12
  • Mar 22 - 26
• Job Setting: Clinic / Outpatient; No Trauma / Not Rated
• Types of Cases: Clinic patients (new and established); Cystoscopies, prostate biopsies, vasectomies; Surgery (both robotic and non-robotic); Adult patient population
• EMR: Cerner
• Credentialing: 60-90 days after receiving a completed application with a clean file. AZ license, MD/DO, and Malpractice Insurance required. CV must be in chronological order (MM/YYYY format). No credentialing with other facilities. Applications must be submitted within 72 hours of receipt.

Minimum Requirements

• Board Certified/Board Eligible: Board Certified in Urology (Required)
• Certifications: ACLS (Required for credentialing), BLS (Required for credentialing), DEA (Required for credentialing)
• Licensure: Active Arizona License or IMLC (Letter of Qualification in hand at time of presentation) (Required)
