• Coverage Area: Kanawha County - travel between patient homes (mileage reimbursed from first to last visit)
• On Call / Weekend Requirement: None

Qualifications / Job Requirements

• Current, unrestricted Physical Therapist license
• Doctoral degree in Physical Therapy (DPT)
• Professional competency in OASIS visits and add-on evaluations (certification or documented experience required)
• Minimum 1 year of experience as a Physical Therapist; home health experience is a plus
• Strong communication, documentation, and organizational skills
• Ability to work independently, manage a flexible schedule, and provide patient-centered care

Responsibilities

• Provide high-quality physical therapy services to home health and hospice patients across Kanawha County
• Conduct OASIS assessments, add-on evaluations, and skilled therapeutic interventions
• Develop and implement individualized care plans to improve patient function, safety, and quality of life
• Document patient evaluations, progress notes, and discharge summaries in accordance with regulatory and agency standards
• Collaborate with interdisciplinary teams (nursing, social work, caregivers) to ensure coordinated, comprehensive care
• Travel between patient homes; follow mileage reimbursement policy (from first patient to last patient)
• Meet productivity expectations (30 points per week) while maintaining clinical quality and patient satisfaction
