Hybrid In Home Nurse Practitioner/Physician Assistant

As an Advanced Practice Clinician (APC), you will be part of a transformative program designed to provide personalized care and support to our patients, including those with complex and chronic needs, all in the comfort of their own homes. The APC will ensure the delivery of high-quality clinical and home-based patient care while promoting optimal health outcomes and providing cost-effective solutions that adhere to Alignment Health's policies, as well as state and federal regulations. You'll assess, develop, and coordinate care options tailored to the needs of our members, who are typically 65 and older.
