Case Manager – Community Health (FQHC) We are seeking a detail-oriented Case Manager to join a mission-driven Federally Qualified Health Center (FQHC) serving underserved communities. Key Responsibilities Provide case management services including care coordination, referrals, and resource navigation Conduct patient assessments to identify medical, behavioral, and social support needs Connect patients with community resources such as housing assistance, transportation services, food programs, and behavioral health support Collaborate with clinical and behavioral health teams to develop and implement individualized care plans Maintain accurate documentation in the electronic health record (EHR) system and track patient progress Support patient engagement, education, and follow-up to improve health outcomes and continuity of care Requirements Certified Peer Support Worker (CPSW) preferred or Bachelor’s degree preferred in Social Work, Public Health, Psychology, or a related field 2+ years of case management or care coordination experience, ideally in an FQHC, community health center, hospital, or social services setting Experience working with underserved or vulnerable populations strongly preferred Knowledge of community resources and social service programs in New Mexico is a plus Strong communication, organizational, and patient advocacy skills Experience with electronic health records (EHR) and care coordination tools preferred Jobot is an Equal Opportunity Employer.
