Job Title Occupational Therapist - Home Health Location Creston, Iowa Schedule / Job Type Contract | Full-Time | Days | Home Health Setting Key Responsibilities
- Evaluate patients in a home health setting to assess functional abilities and develop individualized treatment plans
- Provide occupational therapy interventions aimed at improving patients' ability to perform daily living activities
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to ensure coordinated and comprehensive patient care
- Educate patients and caregivers on therapeutic techniques and home safety strategies
- Current, valid Occupational Therapist licensure in the state of Iowa
- Graduate of an accredited Occupational Therapy program
- NBCOT certification (or eligibility)
- Experience providing occupational therapy services in a home health or similar setting
- Valid driver's license and reliable transportation for home visits
- Prior home health occupational therapy experience
- Familiarity with home health documentation and compliance requirements
- Strong communication and patient education skills
