In-Home Primary Care Physician Assistant

Signify Health is seeking a mission-driven Physician Assistant to perform comprehensive in-home evaluations for diverse, often vulnerable patients. In this role, you will complete detailed histories, physical exams, and diagnostic reviews while identifying care gaps and social determinants of health. You will develop evidence-based care plans, collaborate with physicians and interdisciplinary teams, and use modern technology and analytics to support value-based care.
