Work Environment

This position is primarily onsite, with therapists working directly in patients' homes. The flexible nature of home care means you will experience diverse environments and patient needs daily. Key Responsibilities

• Conduct thorough evaluations of patients to determine their functional limitations and devise evidence-based treatment plans.
• Implement and adjust treatment protocols based on patient progress and feedback.
• Provide consultations to other healthcare professionals regarding patient care strategies.
• Maintain accurate documentation of evaluations, treatment plans, and patient progress in compliance with state regulations and organizational standards.
• Educate patients and their families about rehabilitation goals, treatment methods, and ongoing care.
• Participate in community outreach initiatives to promote health and wellness.
• Supervise and mentor support staff, ensuring high-quality care for all patients.
• Continuously update professional knowledge by attending workshops and in-services related to physical therapy practices.

Patient Population / Client Focus

The successful candidate will work closely with a diverse patient population, including individuals recovering from orthopedic surgeries, managing chronic conditions, or rehabilitating after strokes. Our focus is on providing patient-centered care that respects each individual's unique background and circumstances, ensuring culturally competent services that meet their specific needs.

Qualifications

To excel in this role, candidates must possess the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited program.
• Current state licensure in Physical Therapy.
• Strong written and verbal communication skills, essential for effective interaction with patients, families, and the healthcare team.
• Proficiency in using electronic health record (EHR) systems and standard office software.

Preferred Experience

We seek candidates with a minimum of 2 years of experience in physical therapy, ideally in a home care or outpatient setting. Experience in developing and executing individualized treatment plans, along with a strong understanding of rehabilitation principles, is highly preferred.

Compensation and Benefits

This full-time position offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience.
