Daily Responsibilities: Provide in-home physical therapy services to patients in accordance with their individualized treatment plans Conduct assessments to determine patients' needs and develop appropriate care plans Collaborate with interdisciplinary team members to ensure comprehensive care for patients Educate patients and their families on therapeutic exercises and techniques for ongoing care Maintain accurate and timely documentation of patient progress and treatment outcomes Required Skills: Valid state licensure as a Physical Therapist Experience in manual therapy and therapeutic exercise techniques Ability to develop and implement treatment plans for rehabilitation Excellent communication and interpersonal skills Nice to Have Skills: Knowledge of VitalStim and adaptive equipment Background in speech therapy and gait training Technical Skills: Familiarity with relaxation techniques for patient comfort Salary and Benefits: Salary Range: $115,000 - $125,000 per year Health Insurance Life Insurance Dental Insurance Vision Insurance 401(k) Retirement Plan Health Savings Accounts (HSAs) Flexible Schedules Tuition Reimbursement Holiday Pay Paid Time Off Paid Parental Leave Long-Term Disability Short-Term Disability Company Information: Michigan Community VNA provides home care, hospice, and wellness services, focusing on compassionate care, symptom relief, and preventive health programs for individuals and communities.
