Overview:
Our outpatient primary care practice is seeking an experienced Nurse Practitioner to provide comprehensive, patient-centered care across the lifespan. This role focuses on chronic disease management, preventive care, and long-term patient relationships within a collaborative clinical environment.

Position Offers:
• Monday Wednesday, 9:00 AM 5:00 PM schedule
• Competitive Compensation: $65 $72 per hour (DOE)

• Moderate patient volume
• Stable permanent position with August/September start

Key Responsibilities:
• Deliver comprehensive primary care including well visits, sick visits, and annual physicals
• Manage chronic conditions with a focus on long-term disease and medication management
• Provide ongoing preventive care and patient education
• Serve as an autonomous provider for an assigned patient panel
• Coordinate care with an interdisciplinary team to improve patient outcomes
• Maintain accurate and timely clinical documentation
• Support continuity of care through follow-up and care coordination
• Address complex patient needs in an outpatient setting

Qualifications:
• Minimum of 2 years of primary care experience as a Nurse Practitioner
• Must be credentialed or eligible for Medicare and Medicaid participation
• Comfortable functioning as a sole provider
• Strong clinical judgment and patient management skills
• Compassionate approach to underserved populations
• Available for August/September start
