The Hillside Childrens Center is seeking a Care Coordinator II for the Monroe Region's RIY program. The role delivers coordinated care to youth and families at risk of Juvenile Justice involvement, working with a multidisciplinary team. This full-time position in Rochester includes a Monday–Friday schedule (9:00am–5:00pm) with some evenings/weekends and mileage reimbursements for travel.
