Registered Nurse-Clinical Care Coordinator - NIGHTS/PRN

Job Title: Clinical Care Coordinator Primary Location: Mary Free Bed at Covenant, 700 Cooper Ave, Saginaw, MI Department: Covenant Nursing Employment Type: Per Diem Shift: Nights Compensation: $32.05 starting, increasing with experience. Age–Specific Responsibility Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in the assigned department. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age‑specific needs and to provide the care needed as described in the department's policies and procedures. Summary The Clinical Care Coordinator (CCC) for Mary Free Bed at Covenant is responsible for the overall operation of the rehabilitation hospital and Transitional Care Unit (TCU), and has 24‑hour accountability for all patient care activities and staff who work under his/her direction. The CCC is responsible for organizing, directing, and ensuring effective operations of the rehabilitation hospital and TCU for the assigned shift, taking into account the effect of decisions on the 24‑hour operation of the units. Essential Job Responsibilities Quality Nursing Care: The Clinical Care Coordinators' primary focus is the delivery of quality nursing care. This includes working with all staff to assure that nursing care is delivered consistently according to established policies, procedures, and protocols; creating equitable staffing assignments; mentoring new staff; ensuring nursing presence at and preparedness for team conferences; communicating with, and following up with physicians; safety monitors; timely follow‑up on occurrence reports; and customer service recovery as needed. Review of Nursing Policies: The Clinical Care Coordinator reviews and provides input to the Nurse Manager for nursing policies and procedures, nursing standards of patient care, treatment, and services, and standards of nursing practice; addresses non‑compliance with policies and procedures; and validates key nursing tactics. Key Performance Indicators: The Clinical Care Coordinator understands performance measures within the Rehabilitation Hospital and collaborates with staff to identify opportunities for improvement and potential action plans. When process, policy, or procedure changes are developed by the Program Director or Nurse Manager, the CCC directs staff to the information, assists with implementation, ensures staff complete required competencies, and provides feedback to the Nurse Manager. Quality Improvement: The Clinical Care Coordinator participates in Mary Free Bed at Covenant's quality improvement program and works with staff to provide excellent rehabilitation nursing care through innovative ideas that align with evidence‑based policies and procedures. Patient Safety: The Clinical Care Coordinator plays an important role in providing a safe environment for all patients and staff. As a clinical resource, the CCC evaluates the risks for each patient and, as a patient advocate, ensures appropriate steps are taken to protect patient safety. The CCC also serves as the primary clinical support for employees and responds to all medical emergencies, per policy, as well as all codes (fire, tornado, violence, abductions) within the campus. Interdepartmental and Interdisciplinary Collaboration: The CCC maintains effective interpersonal communication and working relationships with all leaders, staff, physicians, and other key stakeholders; participates actively in interdisciplinary program meetings to evaluate and improve care delivered at Mary Free Bed at Covenant. Accreditation and Regulatory Responsibilities: The CCC assists the Nurse Manager and Program Director with implementing policies, procedures, and processes that align with regulatory, accreditation, and state hospital standards; serves as a resource for staff in these areas. Customer Service: Each CCC demonstrates excellent customer service standards and behaviors while interacting with patients, family members, physicians, and colleagues; deals discreetly and sensitively with confidential information; performs customer service recovery when necessary; and assists staff in recognizing unique cultural and religious needs of patients and families. Professional Relationships: The CCC develops and maintains professional working relationships with colleagues from other disciplines; assists with problem solving and service recovery when challenges arise. Employee Competency: The CCC performs within policies, procedures, and standards of care; demonstrates competency required by the department; assists the Nurse Manager or Program Director with assessment of competencies for new and existing staff; and shares concerns promptly. Professional Work Environment: The CCC encourages a culture of personal accountability, solution‑seeking behavior, mutual respect, open communication, acceptance of change, enthusiasm, and pride; collaborates with management to achieve continuous improvement. Employee Safety: The CCC follows all employee safety policies and procedures, takes appropriate steps to reduce risks, communicates with staff, and maintains a high‑quality nursing environment. Staff Selection, Development, and Mentoring: The CCC assists with employee selection and orientation of new team members, provides professional interview communication, maintains open communication with RN preceptors and the Nurse Educator, and ensures comprehensive orientation for new team members. Student Preceptor and Mentoring: The CCC assigns RN preceptors for students and orientees, oversees care on his/her shift, and ensures a valuable clinical rotation for students. Essential Job Qualifications Licensure: Registered Nurse in the state of Michigan. Education: Associates degree in nursing required; Bachelor's degree preferred; Certification as a Registered Rehabilitation Nurse preferred and encouraged. Basic Life Support (BLS): Current BLS certification required. Experience: Preferred but not required; previous experience as an RN, especially in a rehabilitation or acute care setting, is preferred. Critical Thinking & Leadership: Strong critical thinking, decision‑making, delegation, and leadership skills required. Time Management: Time management and organizational skills essential; effective written and verbal communication skills required. Work under Pressure: Must be able to work effectively under time requirements and deadlines, problem‑solve when facing unexpected issues, handle interruptions, and prioritize effectively.
