If you are passionate about healthcare and providing excellent patient care, we want to hear from you!Position: Patient Care CoordinatorLocation: LaytonType: Part-TimeSchedule: Saturday and Sunday 3pm - 9pmJob Description:As a Care Coordinator at Wee Care Pediatrics, you play a crucial role in ensuring the seamless and coordinated care of our young patients. Your responsibilities include collaborating with medical professionals, facilitating referrals, managing prescription fulfillment, overseeing lab results, serving as a patient point of contact, and ensuring the continuity and quality of care. Job Duties:Provide patients with appropriate referrals to specialists and additional healthcare services.Manage prescription fulfillment and refills, ensuring timely access and accurate records.Receive, document, and communicate lab results for timely follow-up and care decisions.Act as a patient advocate, addressing questions and providing support and information.Coordinate patient care transfers between departments or facilities for continuity of care.Job QualificationsPrevious MA work experience desired; experience in Pediatrics preferred.Understand various lab tests and able to clearly communicate results and provider instructions to parents.Answer basic medical questions and offer advice to parents over the phone ensuring high standard of care.Ability to communicate effectively with providers, staff, and patients/parents.Capable of prioritizing multiple job demands and work in a fast-paced health care environment.Demonstrate compassion and understanding for the patient and families.Basic computer knowledge.Wee Care Pediatrics is an equal opportunity employer.
