Licenses/Certifications: Minnesota Physical Therapist license, BLS (within 1 year)

Experience: Minimum 1 year in health-related field, familiarity with electronic medical records (prefer Epic)

Salary: $80,000 to $115,000 based on experience

What You'll Do

• Provide outpatient and inpatient physical therapy services to a diverse patient population
• Assist with occasional SNF and Home Health care needs
• Document patient progress using Epic electronic medical records
• Collaborate within a dedicated healthcare team focused on quality care
• Contribute to community health initiatives and patient education
• Uphold all safety and documentation protocols

What You Need

• Minnesota Physical Therapist license
• Basic Life Support (BLS) certification (within 1 year of hire)
• At least 1 year of experience in a healthcare setting
• Proficiency with electronic medical records (Epic preferred)

• Experience in outpatient and inpatient rehab settings
• Strong communication and teamwork skills
