Job Summary:

Responsibilities:

• Provide in-home care services to seniors and adults who need support with daily living activities, including bathing, dressing, grooming, and medication management

• Assist with light housekeeping, meal preparation, and laundry

• Maintain a safe and clean living environment for clients

• Develop and implement personalized care plans in collaboration with clients, family members, and healthcare professionals

• Maintain accurate records of client care and services provided

• Communicate effectively with clients, family members, and healthcare professionals to ensure seamless care

Requirements:

• Experience in home health care, hospice, or gerontology preferred

• Ability to lift, move, and transfer clients as needed

• Ability to work independently and as part of a team

• Excellent communication and interpersonal skills

• Ability to maintain confidentiality and handle sensitive information
