Creative Health Solutions is a therapist-operated company providing therapy services at skilled nursing facilities since 2001. Our mission is to develop great clinicians, provide cutting-edge rehab in a caring environment, and support autonomy that respects our therapists' professional clinical decision-making skills. Each employee of CHS contributes to the success of our goal to provide the highest quality patient care in the industry today.
We are seeking a skilled and passionate Physical Therapist to join our team at Creative Health Solutions. The ideal candidate will work closely with a team of healthcare professionals to develop and administer therapy programs aimed at improving patients' physical, mental, and emotional well-being.

Responsibilities
- Develop and administer therapy programs aimed at improving patients' physical, mental, and emotional well-being.
- Work closely with a team of healthcare professionals to create personalized treatment plans for patients.
- Document patient progress and communicate with healthcare professionals regarding treatment plans and patient outcomes.
- Continuously educate patients and their families on therapeutic procedures and techniques
- Stay up to date with the latest therapy techniques and methodologies.
