What You'll Do:

• Administer medications and treatments as ordered

• Monitor residents and report changes in condition

• Maintain accurate and timely documentation

• Assist with admissions, discharges and transfers

• Participate in individualized care planning

• Collaborate with residents, families and interdisciplinary team members

• Follow infection control procedures and maintain a safe care environment

• Provide compassionate, person-centered care

• Support a positive environment for residents, families and staff

Qualifications:

• Active Licensed Practical Nurse (LPN) license in good standing

• Strong communication and interpersonal skills

• Compassionate approach to resident care

• Ability to work collaboratively with residents, families and the healthcare team

• Ability to work effectively in a skilled nursing environment
