Job Description
Southern Jersey Family Medical Centers, Inc. is seeking a talent Registered Dietitian to step into our fast-paced facility!

SUMMARY: The Registered Dietitian (RD) provides comprehensive Medical Nutrition Therapy (MNT) and outpatient nutritional counseling within a primary care setting. Working as an integral member of the multidisciplinary healthcare team, the RD assesses nutritional needs, develops individualized care plans, and delivers evidence-based education to patients managing chronic health conditions (such as diabetes, hypertension, obesity, renal disease, and pediatric nutritional concerns).

DUTIES AND RESPONSIBILITIES

• Patient Assessment & Care Planning:

• Conduct thorough nutritional assessments using clinical data, medical history, eating habits, and socio-economic factors.
• Develop and implement individualized nutritional care plans aligned with the primary care provider's treatment goals.
• Re-evaluate and adjust care plans based on clinical outcomes and patient progress.

• Clinical Counseling & Education:

• Provide specialized Medical Nutrition Therapy (MNT) for patients across the lifespan (pediatric, adult, and geriatric).
• Deliver lifestyle modification counseling focused on chronic disease prevention and management (e.g., Diabetes Self-Management Education).
• Facilitate group nutrition workshops or wellness initiatives for patients and community members.

• Interdisciplinary Collaboration:

• Partner closely with physicians, advanced practice providers, nurses, and care coordinators to ensure seamless patient care.
• Participate in care team huddles, patient case conferences, and quality improvement initiatives.
• Refer patients to internal or community resources (such as food assistance programs or behavioral health) when barriers to care are identified .

• Documentation & Compliance:

• Maintain prompt, accurate, and confidential electronic health record (EHR) documentation of all patient encounters, care plans, and follow-ups.

• Ensure compliance with state/federal regulations, HIPAA, and accreditation standards.
• Complete required billing documentation and coding for nutritional counseling services.

Experience and Skills
POSITION REQUIREMENTS:

• Clinical expertise in Medical Nutrition Therapy (MNT) and chronic disease management (diabetes, hypertension, obesity, renal disease)
• High level of cultural competency and sensitivity to diverse, vulnerable patient populations.
• Proficiency with Electronic Health Records (EHR) documentation and electronic billing/coding workflows.
• Bilingual capability (English/Spanish) preferred.

EDUCATION AND WORK EXPERIENCE

• Bachelor's or Master's degree in Nutrition, Dietetics, or a related field from an accredited college or university.
• Active credential as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) through the Commission on Dietetic Registration (CDR).
• Current State Licensure as a Certified/Licensed Dietitian (if required by state regulation).
• Certified Diabetes Care and Education Specialist (CDCES) preferred (or willingness to obtain).
• Minimum of 1-2 years of outpatient clinical nutrition or primary care experience preferred.
• Experience working within a Federally Qualified Health Center (FQHC) or community health center setting is a plus.
