Peer Support Specialist

Engaging patients remotely, the part-time Peer Support Specialist will build trusting relationships, navigate care systems, and connect individuals with community resources to enhance health outcomes. Key responsibilities Building ongoing relationships with patients and coordinating care to address health needs and barriers Conducting assessments and providing education to support patient engagement and adherence to care plans Documenting interactions and collaborating with healthcare teams to ensure effective care navigation and quality performance Required qualifications At least 3 years of experience in patient navigation, community resource navigation, or a similar role Experience working with patients who have complex medical, behavioral health, or social needs Knowledge of medical terminology, primary care workflows, and social drivers of health Demonstrated ability to work effectively in a remote environment Familiarity with motivational interviewing and care navigation techniques
