Advanced Practice Provider

HarmonyCares seeks an Advanced Practice Provider to deliver home-based primary and palliative care to medically complex, home-limited patients. As part of an interdisciplinary team, you will conduct in-home and telehealth visits, manage chronic conditions, coordinate care plans, and help reduce avoidable hospitalizations. You’ll collaborate closely with physicians, nurses, and care managers in a compassionate, patient-first culture focused on respect, integrity, and access.
