Primary Care PACT Nurse - Hamilton CBOC

The Cincinnati VHA Medical Center is seeking a Primary Care PACT RN at the Hamilton CBOC Clinic. This RN is responsible for providing competent evidence-based, sex-specific care to primary-care patients, overseeing licensed vocational nurses and nursing assistants as appropriate. The 2-page resume requirement does not apply to this occupational series. For more information, refer to the required documents below. Qualifications English language proficiency – required for direct patient care. Graduate of a school of professional nursing approved by the appropriate accrediting agency (ACEN or CCNE). Or individuals in a master’s-level bridge program with equivalent coursework may be eligible to become registered before completion of the bridge program. Upon obtaining a state license, may be appointed on a temporary basis and then converted to a permanent appointment after successful completion of the bridge program. Reference VA Handbook 5005 – Appendix G6. Foreign nursing graduate: must possess a current-full-unrestricted license and, if education is above associate level, a formal equivalency validation accepted by VA (e.g., ICD). Current full-unrestricted license from any U.S. state, territory, commonwealth or District of Columbia. Graduate Nurse Technician exception: candidates who meet basic education but lack licensure may be appointed at entry step for up to 120 days while pursuing licensure, extendable to two years. All VHA employees performing the duties described on 1/29/2024 are considered to have met qualification requirements (grandfathering). Grade and level scope criteria are detailed in the online assessment: Physical requirements: ability to stand, walk, lift up to 35 lbs, perform moderate bending, kneeling, and use safe patient-handing protocols. Ability to pivot waist and neck, push/pull items ≤ 35 lbs, reach within five feet or above shoulder height. Requires adequate vision and hearing; hearing aids permitted. May require flexible schedule. Duties Provide direct veteran care and coordination within the Primary Care Department. Manage assigned panel(s), use registries, and ensure appropriate evaluation and access. Provide health promotion, education, coaching on wellness, disease prevention, and chronic care management. Assist in directing nursing education, orientation, competencies, and quality improvement initiatives. Integrate outpatient nursing knowledge and practice, self-directed goal setting for complex PACT situations. Validate understanding of care processes and participate in evidence-based decision making. Question clinical practices for evidence-based outcomes. Manage competing priorities, demonstrating competence in primary care practice, deliberate planning, and critical thinking. Perform all other duties as assigned. Preferred: Previous primary care or ambulatory experience. Supervisory controls: accountable to the Associate Director of Patient Care Services (ADPCS).
