Hospice Admissions Nurse - Weekend Option

Role Overview: Provides specialized nursing care to hospice patients in their homes or facilities, emphasizing pain management, symptom control, and holistic support. Acts as a case manager when assigned, coordinating all aspects of patient care. Key Responsibilities: Assess patient preferences, needs, and barriers, including cultural and emotional factors. Develop, implement, and document personalized care plans in collaboration with the interprofessional team, patients, and families. Promote respectful, equitable, and empathetic interactions with diverse populations, supporting emotional, spiritual, and cultural needs. Monitor patient condition changes, communicate updates to families and team, and adjust care plans accordingly. Ensure evidence-based practices and quality outcomes through continuous evaluation of nursing care. Qualifications & Skills: RN license with a valid driver’s license. Associate’s degree in nursing (bachelor’s preferred). Less than two years of experience required; 2-5 years preferred for advanced roles. Experience in hospice or home health is preferred. Ability to work safely and maintain alertness at all times. Additional Information: This is a full-time, benefit-eligible role with community travel within the Greater St. Louis Area.
