RN - Home HealthPride Health is hiring an RN - Home Health to support our client's healthcare facility in King County and Snohomish County, WA. Schedule: Monday-Friday | 8:00 AM-4:30 PM | On-Call/Weekend: 5 Weekdays and 2 Weekends Minimum Location: King County and Snohomish County, WA; Mileage reimbursed between patient visits. Job Type: Contract Pay Range: $56-$60 *Pay offered is based on experience, expertise, credentialing, and education.Duties:Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and interdisciplinary healthcare teams.Perform initial and ongoing nursing assessments, including accurate and timely OASIS assessments and medication reconciliation, to determine patient eligibility and medical necessity.Provide skilled nursing care according to the plan of care while supervising and directing home health aides and LPNs in compliance with State and federal regulations.Maintain accurate and timely documentation while coordinating care and communicating with physicians, patients, families, clinicians, and agency staff to ensure quality care and positive patient outcomes.Requirements:License(s): RN license in Washington State or Multi-StateCertification(s): BLSYears of experience: 1+Skills/Specialties/Technology: Home Health experience preferred; HCHB/PointCare experience preferred; OASIS documentation experience preferred; strong clinical assessment, care planning, communication, organization, and time management skills.Equal Employment OpportunityPrideGlobal and it's affiliates are an equal opportunity employer.
