Job Description
The Director of Nursing (DON) is the clinical leader of Project New Generation LLC, holding dual responsibility
across both the AZDHHS-licensed home health program and the DDD service lines. On the home health side, the
DON runs all clinical operations, manages and grows nursing and clinical staff, recruits clients and patients, and
ensures full AZDHHS clinical compliance — operating within the framework of existing policies, procedures, and
protocols already established under the license. On the DDD side, the DON serves as the clinical resource and
medical support partner for the DDD Director and Group Home — stepping in for any medical, nursing, pharmacy,
or health-related needs across the organization. This is a highly collaborative, cross-functional clinical leadership
role that is essential to both the quality of client care and the company's overall success.
1. Home Health Clinical Operations
• Direct and oversee all clinical operations of the AZDHHS-licensed home health program under the
administrative authority of the COO/Administrator.
• Implement, uphold, and keep current all existing clinical policies, procedures, care protocols, and workflows
established under the AZDHHS home health license — updating them as regulations evolve or the
COO/Administrator directs.
• Ensure all home health services are delivered in full compliance with AZDHHS requirements, clinical best
practices, and company standards.
• Maintain survey readiness at all times — ensuring clinical records, care plans, and documentation are
complete, accurate, and audit-ready.
• Oversee clinical quality assurance — conducting regular chart audits, monitoring outcomes, tracking incidents,
and implementing corrective action as needed.
• Manage infection control protocols, clinical risk management, and patient safety initiatives within the home
health program.
2. Client & Patient Recruitment — Home Health• Actively recruit new home health clients and patients by building relationships with hospitals, discharge
planners, physicians, specialists, skilled nursing facilities, and community referral sources.
• Represent Project New Generation LLC's home health services at community events, healthcare networking
functions, and outreach activities.
• Manage the clinical intake and evaluation process for new home health patients — conducting or overseeing
initial nursing assessments and ensuring eligibility and authorization requirements are met.
• Develop relationships with physician offices and medical practices to establish a consistent referral pipeline for
home health services.
• Collaborate with the COO and DDD Director to identify existing DDD clients who may also qualify for home
health services, maximizing care coordination across both programs.
3. Nursing Staff Management & Pairing
• Recruit, hire (in coordination with the COO), onboard, supervise, schedule, and evaluate all nursing and
clinical staff including RNs, LPNs, CNAs, and home health aides.
• Strategically pair nurses and clinical staff with clients and patients based on clinical needs, skill set, availability,
personality compatibility, and geographic proximity — ensuring the best possible match for client outcomes and
staff satisfaction.
• Conduct regular performance evaluations for all clinical staff — identifying strengths, addressing deficiencies,
setting professional development goals, and documenting all findings.
• Ensure all nursing and clinical staff maintain current licensure, certifications, required training hours, and
background clearances.
• Provide ongoing clinical supervision, mentorship, and support to nursing staff to promote professional growth
and high-quality care delivery.
• Address clinical performance issues promptly and in coordination with the COO for any HR-related actions.
4. Client Clinical Evaluations & Care Management
• Conduct or oversee comprehensive nursing assessments and evaluations for all home health clients upon
intake, at scheduled intervals, and whenever a change in condition occurs.
• Develop, implement, review, and update individualized care plans in collaboration with the client, their family or
guardian, and the treating physician.
• Monitor client health status, track clinical outcomes, and ensure continuity of care across all service
interactions.
• Coordinate with external healthcare providers including physicians, specialists, therapists, and pharmacists to
ensure integrated, whole-person care.
• Ensure all physician orders are current, properly documented, and being carried out correctly by clinical staff.
• Manage high-acuity or complex client cases directly, providing hands-on clinical oversight and intervention as
needed.
5. DDD Medical & Clinical Support — Cross-Functional Role
• Serve as the clinical resource and medical support partner for the DDD Director and Group Home —
collaborating closely to address any health, nursing, or medical needs that arise on the DDD side of the
company.• Step in for any medical evaluations, nursing assessments, health consultations, or clinical documentation
required for DDD clients in the Group Home or any other DDD service line.
• Work hand in hand with the DDD Director on Group Home compliance — specifically any medical, clinical, or
health-related components of DDD requirements including health and safety standards, nursing oversight
requirements, and medical recordkeeping.
• Oversee or coordinate medication management and pharmacy-related needs for DDD clients — including
medication administration protocols, pharmacy communication, prescription coordination, medication
reconciliation, and staff training on medication procedures.
• Collaborate with the DDD Director and professional staff on any clinical or medically-driven components of
Individual Support Plans (ISPs) — contributing nursing expertise to health goals, medical action plans, and care
coordination.
• Provide clinical guidance and support to DDD direct care staff and habilitation workers when client health
situations arise — serving as the go-to clinical resource for the entire team.
• Coordinate with external providers (physicians, specialists, therapists) on behalf of DDD clients as needed,
ensuring that medical needs are addressed promptly and documented appropriately.
• Conduct health-related evaluations and assessments for DDD clients as required by DDD compliance
standards or as requested by the DDD Director or COO.
6. Pharmacy & Medication Coordination
• Establish and maintain working relationships with pharmacy partners to ensure timely and accurate medication
fulfillment for both home health and DDD clients.
• Ensure all medication administration records (MARs) are current, accurate, and properly maintained across all
applicable service lines.
• Oversee medication error reporting, adverse event tracking, and corrective protocols for medication-related
incidents.
• Train and ensure competency of staff involved in any aspect of medication support, administration, or
documentation.
• Coordinate prescription renewals, prior authorizations, and medication changes with treating physicians and
pharmacies.
7. AZDHHS Clinical Compliance
• Maintain full clinical compliance with all AZDHHS home health regulations on an ongoing basis — keeping
clinical policies, procedures, and documentation practices current with regulatory requirements.
• Prepare and lead the clinical team through AZDHHS surveys and inspections — ensuring all clinical records,
protocols, and staff credentials are survey-ready.
• Develop and implement corrective action plans for any clinical deficiencies identified during surveys, audits, or
internal reviews.
• Report clinical compliance status, outcomes data, and quality metrics to the COO/Administrator on a regular
basis.
• Ensure all required AZDHHS clinical reporting is completed accurately and submitted on time.
8. Home Health Business Development & Growth• Actively contribute to the growth of the home health program by building a strong referral network and
consistently recruiting new patients.
• Identify opportunities to expand service offerings, add clinical staff capacity, or enter new patient populations
within the scope of the AZDHHS license.
• Collaborate with the COO on strategic planning for the home health program — providing clinical insight on
feasibility, staffing needs, and service line expansion.
• Track home health census, patient outcomes, and program performance metrics — reporting to the COO and
contributing to revenue growth goals.
• Work with the DDD Director to ensure clients receiving both DDD and home health services are identified and
appropriately enrolled in both programs.
9. Qualifications & Requirements
• Current, unrestricted Registered Nurse (RN) license in the State of Arizona — required.
• Minimum 3 years of clinical nursing experience; home health or community-based care experience strongly
preferred.
• Prior nursing leadership, supervisory, or management experience preferred.
• Working knowledge of AZDHHS home health regulations and clinical compliance standards.
• Experience with DDD clients, developmental disabilities, or HCBS settings preferred.
• Strong clinical assessment, care planning, and documentation skills.
• Demonstrated ability to lead and develop a clinical team.
• Excellent communication, collaboration, and organizational skills.
• Experience with pharmacy coordination and medication management protocols.
• Must pass all required background checks and maintain licensure and clearance throughout employment.
$186 per hour
